Please fill out one form per dog.

  • Group classes fill quickly. We will contact you ASAP upon receiving your registration. 
  • All dog's MUST complete Basic Obedience and /or have permission from Danielle to participate in Agility. Please request a free evaluation if you are unsure where your dog is best suited.
  • All dogs must be up to date on all shots and we will require proof from your veterinarian.
